Reggie's Sleepout Minor Consent and Fundraising Forms

Young people under the age of 18 should have a minor consent form filled out and signed by a parent or guardian.  The minor consent form should be returned to the registration table located inside the Field House at the time of the event between 4 p.m. to 8 p.m.  In the table below, there are other forms that may be downloaded to give you examples of how to ask your friends and family members to support you.

Minor Consent Waiver Form	Anyone 16 and younger will need this form filled out at time and bring to the registration table at the event.
